Assisted Living

Each resident’s care plan is personalized to ensure activities, independence, and assistance are all balanced for a healthy lifestyle. Some of our offerings include assistance with the following:

Assisted living provides your loved one with the opportunity to interact with peers through day-to-day relations, planned activities, and events. Residents can focus on socializing, having fun, and building relationships while still feeling at ease knowing support is available if needed. While still valuing the independence of our residents, we always want to assist them in living their most comfortable lives.

Skilled and Intermediate Nursing Care

All residents are provided daily nursing services that are directed by physician’s orders and treatment plans.

Skilled nursing provides short-term care to residents focused on recovery and rehabilitation so they can return to their normal lives as quickly as possible. These services are often utilized during recuperation from serious health issues and are not able to be cared for at home.

Intermediate care is utilized for residents who do not require daily medical treatment but are unable to sufficiently care for themselves at home. This option provides light nursing care and assistance with daily activities. This is often the type of care that is pursued when looking for a balance of independence with an appropriate level of care, found in nursing homes.

Physical therapy aims, to help restore and improve functionality, reduce pain, and increase mobility for better strength and balance. Physical therapy can help make daily tasks easier, give residents a greater sense of independence, and improve confidence.

Speech Therapy

The ability to communicate effectively is important for everyone, no matter their age. Speech therapy is often necessary when our resident is recovering from the debilitating consequences of a stroke, dementia or a head injury. Working with a speech pathologist can improve a resident’s ability to interact with others, feel more confident and less isolated. Areas of concentration include:

Our Experienced Physicians

Dr. Mark Evans, Medical Director

Dr. Mark Evans is a native of Richwood, Ohio,  graduating from North Union High School in 1976 before receiving his Bachelor of Arts degree at Ohio Northern University in Ada, Ohio. Dr. Evans received his medical degree from the Ohio State University College of Medicine, completed a three-year residency training in family medicine at Mt. Carmel Hospital, and, was board-certified in family medicine in 1987.

Dr. Evans concentrated on geriatrics while working at the Gerlach Center at Riverside Methodist Hospital from 2000 to 2001. He has since been affiliated with several geriatric-related geriatric-related facilities, acting as Medical Director for several geriatric related modalities in Central Ohio, including home healthcare companies. hospice agencies and intravenous infusion agencies.
He is currently an Associate Professor of Medicine at Ohio State University while also serving as the medical director at assisted living and nursing facilities around Columbus. Dr. Evans maintains active privileges at Mt. Carmel Medical Center.

Among his many hobbies, Dr. Evans enjoys music, playing piano, golfing, and supporting his favorite sports team, The Buckeyes. He is a member of the Lifepoint Church in Hilliard, Ohio. Dr. Evans, a widower, is a proud father to Abby and Nathan and is thrilled to be a first-time grandfather.

Dr. Theresa Calimlin, Physiatrist

Dr. Theresa Calimlin graduated summa cum laude from the University of Cincinnati in 2000. She attended the Ohio State University Medical School where she also completed her physical medicine and rehabilitation residency. In addition to being board-certified in Physical Medicine and Rehabilitation, Dr. Calimlin is also board-certified in Electrodiagnostic Medicine. Dr. Calimlin held a faculty position at the Ohio State University Medical Center before joining U.S. Physiatry in 2014.

U.S. Physiatry is a national organization of physicians specializing in physiatry or physical medicine and rehabilitation. A physiatrist’s expertise is in treating patients with traumatic brain injury, spinal cord injury, chronic pain, and those who have suffered a stroke or amputation. They enhance and restore functional ability and quality of life in those with physical impairments and disabilities.

Physiatrists are dedicated to providing the highest level of care across the rehabilitation continuum, allowing the patient to reach their maximum potential and improve their of quality of life. Dr. Calimlin is the cornerstone for the Columbus branch of U.S. Physiatry and the organization’s ambassador.
