Assisted Living

Each resident’s care plan is personalized to ensure activities, independence, and assistance are all balanced for a healthy lifestyle. Some of our offerings include assistance with the following:

Assisted living provides your loved one with the opportunity to interact with peers through day-to-day relations, planned activities, and events. Residents can focus on socializing, having fun, and building relationships while still feeling at ease knowing support is available if needed. While still valuing the independence of our residents, we always want to assist them in living their most comfortable lives.

Skilled and Intermediate Nursing Care

All residents are provided daily nursing services that are directed by physician’s orders and treatment plans.

Skilled nursing provides short-term care to residents focused on recovery and rehabilitation so they can return to their normal lives as quickly as possible. These services are often utilized during recuperation from serious health issues and are not able to be cared for at home.

Intermediate care is utilized for residents who do not require daily medical treatment but are unable to sufficiently care for themselves at home. This option provides light nursing care and assistance with daily activities. This is often the type of care that is pursued when looking for a balance of independence with an appropriate level of care, found in nursing homes.

The therapy team prepares an individualized plan of care for each resident to achieve their maximum functional level and to move towards recovery.

Physical Therapy

Physical therapy aims, to help restore and improve functionality, reduce pain, and increase mobility for better strength and balance. Physical therapy can help make daily tasks easier, give residents a greater sense of independence, and improve confidence.

During physical therapy sessions, therapists work with residents based on their plan of care. Areas of concentration include:

Occupational Therapy

Speech Therapy

The ability to communicate effectively is important for everyone, no matter their age. Speech therapy is often necessary when our resident is recovering from the debilitating consequences of a stroke, dementia or a head injury. Working with a speech pathologist can improve a resident’s ability to interact with others, feel more confident and less isolated. Areas of concentration include:

Dr. Rachel Hunter

Rachel Hunter, DO, is a hospitalist and a board-certified Internal Medicine physician specially trained in the care of hospitalized patients, with a focus on the geriatric population at The Sanctuary at Wilmington Place.

She will be your primary care physician who will ensure the highest level of care during your stay. Dr. Hunter is committed to providing exceptional, around-the-clock specialized care tailored to the unique needs of our residents.
